위기에 처한 가족을 위한 간호의 필요성이 점차 증가되어 가족 적응 연구의 중요성이 증대되고 있다. 이에 본 연구는 McCubbin과 McCubbin(1993)에 의해 개발된 가족 적응 이론인 Resiliency Model of Family Stress, Adjustment and Adaptation(이하 Resiliency Model)을 이론적 기틀로 하여 만성 신부전 환자가 인지한 가족 적응의 결정 요인을 확인하기 위한 목적으로 시도 되었다. 연구 결과 만성 신부전 환자가 인지한 가족적응은 가족의 누적요구와 가족 힘에 의해 결정되는 것으로 확인되었으며 그 경로는 누적 요구가 가족 힘을 통하여 가족 적응에 영향을 미치는 것을 알 수 있다. 가족의 누적 요구가 클수록 가족 힘이 작고 가족 힘이 클수록 가족 적응이 작았으며 가족 적응에 미치는 효과를 보았을 때 누적 요구보다는 가족 힘이 더 커다란 효과를 지니는 것으로 밝혀졌다.
This study, based on the Resiliency Model, was performed to confirm the factors which determine the family adaptation as perceived by patients on homodialysis.
The tools used in this study were the Family Inventory of Regenerativity and Adaptation-General (FIR.A-G). Data collection were done from January 23, 1995 to March 28, 1995. The 198 hemodialytic patients participated in this study, and SPSS PC+ was used for analysis of data and to examine the model/ data fit, LISR.EL -7.16 analysis (Joreskog & Sorbom, 1989) was done.
The results are as follows
1. Family strength had a significant direct negative effect on family adaptation (V -3.984, T= -3.305).
2. Pile up stress had a significant direct negative effect on family strength (7=-.437, T=-2.781).
3. Model/data fit was relatively good [Chi-square= 40.22 (df= 12, p< .01), Goodness of Fit Index= .937, Adjusted Goodness of Fit Index= .864, Root Mean Square Residual= .063.
In conclusion, family strength and pile up stress were confirmed as factors which determine the family adaptation for patients with chronic renal failure.
